We’ve been providing quality care to individuals and families in Northern Virginia for nearly 40 years. While we are well versed in the most state-of-the-art dental techniques and services, we also have the patience, understanding and empathy that only an established practice can offer. We are ON CALL 24 hours a day for emergencies. We are also proud to offer evening hours to accommodate your busy schedule. Patient Forms In order to make your visit as easy, comfortable and stress-free as possible, we’ve enabled you to download the HIPAA Privacy form, Patient Information Form and Child Patient Information Form located to the right. Please feel free to download and print out these forms, fill them out and bring them to the office in order to make your visit as simple as possible. Insurance and Payment We will be glad to submit our patient’s insurance claim for the treatment performed. It is our policy that patients are expected to pay their portion of the charges at the time services are provided. Our staff will be happy to file insurance claims and pre-estimates as a courtesy to our patients. We will do our best to help you maximize your benefits; however, you are responsible for any amount not covered by your insurance. Vienna Family Dentist accepts cash, checks, Care Credit, MasterCard and Visa. Emergency Care Should you have a dental emergency, we are here to help. Your dentist should be the first person you call if you have a dental emergency. If the office is closed, our answering message will provide you with a phone number to access emergency care 24 hours a day. Be sure to keep your dentist’s after hours contact information available at all times. Whether at home or traveling, the following tips can help you manage a dental emergency until you can get to the dentist. It is important to remember that with some dental emergencies, seeing a dentist within 30 minutes or less can mean the difference between saving or losing your tooth.
